The clinical rotations take place at one of the eleven rural and urban community health center sites that have been approved as Illinois SEARCH program sites. The SEARCH program coordinator will work with the individual students/residents to arrange the clinical rotation placement. Placement availability at each center varies, therefore placement at a requested site cannot be guaranteed.

- Access Community Health Network/Rogers Park Family Health Center (Chicago-Rogers Pk)- Spanish-speaking preferred
- Central Counties Health Center (Springfield)
- Chicago Department of Public Health Mental Health Center (Chicago) - for Behavioral Health rotation only
- Chicago Family Health Center (Chicago - S. Chicago)
- Crusader Clinic (Rockford)
- Erie Family Health Center (Chicago - W. Town & Humboldt Park) - Spanish-speaking required
- Greater Elgin Family Care Center (Elgin)
- Lake County Community Health Center (Waukegan)
- PCC Community Wellness Center(Oak Park)
- PrimeCare Community Health (Chicago - W. Town) - Spanish-speaking required
- Lawndale Christian Health Center (Chicago - Lawndale)
- Shawnee Health Services (So. Illinois/ Carbondale area)
- Southern IL Healthcare Foundation (E. St. Louis area)
